NAFLD often starts silently due to excess sugar like fructose, which the liver converts directly to fat, leading to steatosis without initial inflammation. Progression to NASH involves escalating damage from ongoing sugar overload, inflammation, and fibrosis, doubling diabetes risk in affected individuals.

NAFLD Symptoms

Early NAFLD (simple fat buildup) is typically asymptomatic, though some notice fatigue, mild right-upper abdominal discomfort, or unexplained weight gain around the midsection.
No overt signs appear in most cases until imaging or blood tests detect elevated liver enzymes like ALT/AST.

Progression to NASH

Sugar-driven NAFLD advances to NASH when fat accumulation triggers liver inflammation and cell injury, fueled by fructose's role in de novo lipogenesis and insulin resistance.
This shift occurs in 20-30% of NAFLD cases, worsened by metabolic syndrome; NASH brings persistent tiredness, pain under ribs, and potential fibrosis scarring.
