Why Was Sweet Urine Historically Linked to Diabetes?

Before modern lab tests, doctors diagnosed diabetes by observing symptoms—and one of the most striking clues was urine that tasted or smelled sweet. This “sweet urine” observation became the defining feature of the disease and even gave it the name diabetes mellitus.

Ancient Observations: “Honey Urine”

  • In ancient India (around 5th–6th century BCE), physicians Charaka and Sushruta described a condition called madhumeha—literally “honey urine”—noting that the urine of affected patients attracted ants and flies due to its sweetness.

  • Similar descriptions appear in Egyptian, Chinese, and Greek medical texts, where excessive urination (polyuria) and sweet-tasting urine were recognized as hallmark signs.

The Term “Diabetes Mellitus”

  • The word diabetes comes from Greek, meaning “to pass through” or “siphon,” describing the excessive urination.

  • In 1674, English physician Thomas Willis rediscovered the sweet taste of diabetic urine in Europe and added the Latin term mellitus (“honey-sweet”) to distinguish it from other causes of polyuria.

  • This is how the condition became known as diabetes mellitus—“the sweet siphon.”

Scientific Confirmation: Sugar in Urine and Blood

  • In 1776, physician Matthew Dobson experimentally proved that the sweetness came from sugar in the urine. He evaporated diabetic urine and obtained a sugary residue, confirming the presence of sugar.

  • Dobson also showed that sugar was present in the blood before appearing in urine, laying the foundation for understanding hyperglycemia (high blood sugar) as the root cause.

Why Does Urine Become Sweet in Diabetes?

In diabetes, the body either doesn’t produce enough insulin or can’t use it effectively. This leads to:

  • High blood glucose levels (hyperglycemia)

  • When blood sugar exceeds the kidney’s reabsorption threshold (~180 mg/dL), excess glucose spills into the urine (glycosuria)

  • Glucose in urine makes it taste or smell sweet and can attract insects.
